COVID-19 Response: CHT Resources and Tools
Based on the virus’s timeline, newly emerging priorities and global best practices, our community has designed a set of modular tools powered by the Community Health Toolkit for COVID-19 preparedness, surveillance, and response. These tools work together as an integrated platform for COVID-19 response and community health, and can be rapidly configured to meet a community’s needs.
Port of Entry Screening App
For many countries, early detection and surveillance interventions are key to mitigating the impact of COVID-19 through proactive identification and treatment of patients before increased local transmission. The Port of Entry Screening App:
Supports enrollment, screening, and self-isolation follow up of all incoming travelers at ports of entry. It can be integrated with Event-Based Surveillance, Contact Tracing and Proactive messaging workflows.
Is based on MoHP Nepal and WHO guidelines, and is currently deployed in partnership with Nepal’s Ministry of Health and Population

Event-Based Surveillance System
Event-Based Surveillance (EBS) allows communities to achieve the earliest possible detection of COVID-19 cases by providing centralized visibility on signals, reports, and investigations. The EBS System is:
A digital surveillance system for the community and at facilities to report, verify and investigate suspect cases by SMS or an android app
Designed based on an existing EBS platform deployed by MoH Kenya and CDC for Integrated Disease Surveillance and Response
Able to monitor other signals prioritized during the pandemic, including neonatal tetanus, acute flaccid paralysis, and measles/ rubella

Contact Tracing
Contact tracing facilitates the registration of COVID-19 patients and their exposed contacts within communities to ensure symptom screening, self-isolation, and referrals to care for those exposed in order to reduce transmission and prevent secondary spread of the virus. Contact Tracing:
Supports tracing of contacts of suspected patients, symptom screening, referrals, and monitoring of asymptomatic contacts during self-isolation
Works on SMS and the android app

Rapid Diagnostic Testing App
Patient care for COVID-19 requires not only ongoing community-level surveillance but also CHW-guided symptom screening, testing, and referrals to care. The RDT App is:
App to guide CHWs through COVID-19 rapid diagnostic testing and referral processes, and share protective measures for health workers that are conducting testing
Based on Partner’s In Health’s protocol for the Antibody (IgM/IgG) test, but can be adapted for different rapid tests
Designed to:
Improve quality of testing and care
Help health workers stay safe
Improve patient follow-up
Support patient education
Be adapted as needed and integrated into existing apps and surveillance platforms.
